Template Chain of Custody Form that Anti-Doping Organizations (ADOs) can use as part of their doping control (sample collection) programs. The Chain of Custody form is used to track the chain of custody from the time a sample is sealed until it is transferred to a WADA-accredited or WADA-approved for the Athlete Biological Passport (ABP) laboratory.
